The Clinical Pharmacy Specialist -Ambulatory position is located in a new Transitions of Care Clinic in Indianapolis, Indiana.

Job Duties
  • The Clinical Pharmacy Specialist-Ambulatory works closely with physicians and health care teams, while providing direct patient care after hospital discharge
  • Will help establish the service provided by the pharmacy team in this setting
  • Will meet with patients in dual appointments with a collaborating physician to provide extensive drug and disease state education and assessment with a particular focus on patients at high risk for hospitalization or poor health outcome.
  • Common disease states include, but are not limited to, diabetes, COPD, and CHF. Additional areas of focus include infection disease and complex co-morbidities in a high-risk population.
  • Will work closely with patients and pharmacies to overcome medication adherence and medication access issues• With the assistance of medication assistance coordinators, assist patients in obtaining cost effective medications
  • Will serve as a drug information resource for the other providers and office staff, answering medication therapy questions
  • Monitors for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests.
  • Engages in population health projects to impact the lives of patients we serve
  • Precepting of pharmacy students and residents

Completion of a PGY2 residency with ambulatory care focus or internal medicine with ambulatory care experiences is strongly encouraged or a minimum of three (3) years working under a drug therapy agreement in a clinical setting

Graduate of accredited college of pharmacy required

Valid Indiana Pharmacist license, Board Certification is expected within 2 years of working in this position
