Job Location The Clinical Pharmacy Specialist position is located in Indianapolis, Indiana.

  • The Clinical Pharmacy Specialist Ambulatory works closely with physicians and health care teams, while providing direct patient care
  • Using a collaborative drug therapy agreement, the Pharmacist will meet with patients individually to provide extensive drug and disease state education with a particular focus on patients at high risk for hospitalization or poor health outcomes
  • Will optimize medication regimens for patients with a variety of conditions, such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis
  • Will work closely with patients to overcome medication adherence issues
  • Uses various strategies to lower patient medication expenses
  • Will serve as a drug information resource for the Physicians and nursing staff, answering medication therapy questions
  • Monitors for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests. 
  • In addition to supervising pharmacy students and residents, this position may also supervise medical assistants and front office staff

Experience

Completion of a PGY2 residency with a cardiology or ambulatory care focus is strongly encouraged.


Education
Graduate of accredited college of pharmacy required


License
Valid Indiana Pharmacist license
Board Certification is expected within 2 years of working in this position
