Job Location 
The Case Manager position is located in Indianapolis, Indiana.

Job Duties
The Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services.
The Case Manager is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.

Experience
Master’s Degree with three years of recent clinical experience preferred.

Education

Master’s Degree in Social Work required

License

Currently licensed by the State of Indiana as a Social Worker required.
Certified Case Manager preferred.

  • Excellent communication, interpersonal, strong organizational, problem solving, decision making, and technical skills
  • Ability to establish rapport with a wide variety of people
  • Exhibit openness to change and willingness to try new approaches to problem solving
Additional InformationThis position is part-time, working 48 hours every two weeks.  3 days per week 8am-4:30pm, these days include every other weekend. On the weekend days in house from 8am-4:30pm on call via phone from 4:30pm to 11pm.  This position is benefits eligible.
