Job Location
The Care Coordinator role will be community based traveling from Shelbyville, Indiana to Indianapolis, Indiana.

Job Description
Provides collaborative planning, referral and monitoring of treatment and support services to adults with severe mental illness, persons with chronic addictions, or children with severe emotional disturbances and their families.

Bachelor’s Degree plus (2) years’ experience in mental health related field preferred

Bachelor’s degree in mental health related field

Current Indiana Driver’s license and proof of auto insurance required; driving record that adheres to CHI policy

List of Duties
  • Apply/follow up with Social Security Benefits
  • Apply/follow up with BDDS, Wrap Around, and other special care services
  • Housing assistance (search and link to resources, apply for services, budgeting)
  • Medication management: assist family in ensuring attend mental health medication appointments, obtain medication, monitor client’s progress with medication, and provide education to client/family regarding mental health medications.
  • Link to extracurricular activities for client and family
  • Link to family support services
  • Skills training with parents (financial literacy, medical care, basic daily living skills, parenting skills etc.)
  • Skills training with youth as needed and identified from therapist and family
  • Support and advocate between therapist, school, and parent
