Job Location
The Care Coordinator role is located across from Community Hospital East.

Job Duties
The Case Manager is responsible for working with Veterans and their families in order to locate, obtain and maintain permanent housing.

Experience
Bachelor’s Degree plus (2) years’ experience in case management required

Education
Bachelor’s degree in health care, mental health care, health-care administration related field preferred

Skills
  • Must have Valid Indiana Driver’s License and reliable transportation
  • Must have effective interpersonal and oral communication skills
  • Must have good written communication skills
  • Must be proficient with computers and use of basic software programs such as Microsoft Office
  • Must be proficient in use of cell phones and other applicable electronic equipment
  • Excellent organization, time and task priority management skills
Additional Information
In addition, the Case Manager works with Veterans, their family and the Housing, Resource and Outreach Specialist to work as a team in order to determine and coordinate additional supports as needed.
