Job Location
The Care Coordinator role will be located at the Washington Pavilion within the Pediatrics department.

Job Duties
Provides collaborative planning, referral and monitoring of treatment and support services to adults with severe mental illness, persons with chronic addictions, or children with severe emotional disturbances and their families.

Experience
Bachelor’s Degree plus (2) years’ experience in mental health related field preferred

Education
Bachelor’s degree in mental health related field

  • Must have the ability to relate in a non-judgmental and culturally competent manner
  • Must have interpersonal skills including motivation, flexibility, persistence, and patience
  • Must value client/family centered care and able to apply strengths based needs assessment
  • Must have the knowledge of behavioral health competencies specific to population being served
  • Must possess the ability to function independently under supervision, problem solving abilities/good judgment
Additional Information
The Care Coordinator role will service our pediatric population. The main function of this role is to address barriers that impact the families we serve. The new hire will be responsible for connecting patients to resources and or assisting families with navigating different systems.
