Job Location
This advanced practice provider will join Community Health Network, a multispecialty group with over 1,100 providers. This opportunity will be focused on the well-being of onsite employees and jockeys at our Indiana Standardbred Clinic in Anderson, Indiana.

Highlights
Community Physician Network, part of Community Health, has an excellent opportunity for a part-time family medicine trained advanced practice provider to work at our Indiana Standardbred Onsite Clinic, dedicated to acute primary care needs.

• Part-time position: 24 hours/week
• Practice hours are Tuesday, Wednesday, and Friday from 8:30 am – 4:30 pm
• Support staff on-site
• No on-call, weekend, or holiday requirements
• Electronic Medical Record (EPIC)

Experience
Minimum three to five years of experience as an advanced practice provider is firmly required. This is an autonomous provider work setting.

Job Duties
Advanced practice provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Education
Educational background consistent with requirements for licensure and certification with the State of Indiana
