This experienced nurse practitioner or physician assistant will join Community Health Network, a multispecialty group with over 1,100 providers in Indiana. This internal medicine opportunity will be located at our Kokomo Internal Medicine Care practice at: 2401 Columbus Boulevard, Kokomo, IN 46901.
Community Health Network has an excellent opportunity for a board certified, internal medicine or family medicine trained advanced practice provider to work at our outpatient Internal Medicine Care practice on our Community Howard Regional Hospital campus: the healthcare provider of choice in North Central Indiana.

Minimum 3 to 5 years of nurse practitioner or physician assistant experience in internal medicine or family medicine is firmly required.
Nurse practitioner or physician assistant will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.
