Personal relationships

What can you expect from our patient-centered medical home model? You’ll enjoy a personal relationship with a healthcare team that is dedicated to meeting all of your heath care needs, no matter your age or your stage in life.

The members of your individual team will spend time getting to know you, and will happily respond to all your concerns and questions. That’s a good thing, because research has shown that patients who communicate with their doctors are not only happier with the care they receive, they actually have better outcomes. Remember, every member of your team has one common goal: to provide you with the best possible healthcare.

Convenient access to care

Most of our practices offer same-day and next-day appointments, after-hours phone access and extended hours—early mornings, evenings and Saturdays. You’ll also have increased access to more providers and resources within our medical neighborhood with our patient-centered medical home model. That ensures you’ll have comprehensive care and continuity of care. As you require additional services from specialists and other healthcare providers, they’ll become part of your medical team.

Electronic medical records

Easy access to your electronic health record through MyChart will give each member of your team a complete, up-to-date picture of your health, so your care will be coordinated, no matter how many healthcare providers make up your personal team.

Patient responsibilities

As the center of your team, and its most important member, you are a full partner when it comes to your healthcare. It is your responsibility to supply the members of your team with the information they need to do an optimal job.

For your convenience, here are some checklists of things to do before, during and after your appointment to maximize your experience.

Before your appointment

  • Gather all the medications you are currently taking, if at all possible. This includes prescription medicines and over-the-counter medicines such as aspirin or antacids, vitamins, dietary or herbal supplements. If bringing your medications is not possible, be sure to write down the name and how much you take of all your medications.
  • Make a list of all the doctors and clinics you see and bring it with you.
  • Write down all the questions you want to ask during your appointment. You might have questions about specific medicines, medical tests, procedures or surgeries, or lifestyle issues such as nutrition or exercise.
  • List your questions so that the most important ones are at the top to ensure you’ll get the answers you need.
  • Be prepared to provide information about your current medical conditions, past surgeries and illnesses, including dates. You’ll also want to be familiar with your family’s medical history. This information can be critical when it comes to providing you with the best possible care.
  • Write down the names and contact information for all members of your team, and keep it up-to-date. You’ll want to take the list with you to your appointment.
  • Be familiar with your health insurance coverage, and contact your insurance provider if you have any questions about your benefits.
  • Call to reschedule or cancel appointments as soon as possible.

During your appointment

  • Show all of your medications, in their original, labeled containers, to your provider if possible. If bringing your medications is not possible, show the list you made with each of your medication names and how you take it.
  • Mention any home remedies you might have used to treat your condition(s).
  • Give your list of all the doctors and clinics to your provider so we can be sure to coordinate your care well.
  • Make your health care provider aware of any changes in your health and your condition(s) since your last visit. Also tell us about changes in the health of any of your family members. It will help us take better care of you!
  • If you have an existing health care plan, discuss what components of it have worked well for you and what components need tweaking. Your health care provider can adjust the plan to fit your individual needs.
  • Discuss your health issues and be sure you understand what you need to do for each one. Ask your health care provider which one(s) you should work on first.
  • Go through your list of questions, beginning with your most urgent or important ones. Be sure to ask how you can reach your care team after hours.
  • Be sure you understand the instructions your provider gives you before you leave the office. We don’t mind repeating or using different words to explain!

After your appointment

  • Continue to learn about your condition(s) and what you can do to stay as healthy as possible.
  • Follow the plan that you and your health care provider discussed. If you have ANY questions about your treatment plan, be sure to call us. We want you to understand how to take the best care of yourself.
  • Take your medications as your provider ordered. If you were advised to stop medications, including any supplements, vitamins or over-the-counter remedies, be sure to stop taking them! If you have ANY questions about your medicines, be sure to call us.
  • Call us if you haven’t received test results within the time your provider told you to expect your results.
  • Contact us after hours if your issue can’t wait until the regular office hours. We can still help direct your care.
  • Pay your share of any fees incurred for your care.
  • Tell us how we are doing! We want to hear from you. You may receive a satisfaction survey in the mail. If you do, please fill it out and send it back! It will help us improve your care and make us better at what we do.
