Cancer Survivorship Program

Survivorship begins from the day of diagnosis. However, it is when the active phases of your cancer treatment are complete that a new normal emerges. Community Health Network's Survivorship Program provides awareness of recommended screenings, education on evidence-based lifestyle behaviors, and assistance with any late- and long-term side effects that may have resulted from cancer treatment. Preventive care is important during this time, and we empower you with the tools to succeed in this next step. The Survivorship Program keeps patients connected to their current cancer care team at Community while integrating the primary care physician. We offer a variety of additional resources and specialty care programs focused on improving the cancer survivor’s health and quality of life.

Thriving After Cancer - Virtual Workshops

Virtual Fitness Classes for Cancer Survivors

  • Survivorship fitness class optionsCardio Strength Training - This cardio circuit workout class includes both free weight and body weight exercises. Modifications offered.
  • Guided Meditation - This relaxation technique uses breath and mental images to bring you to a place of peace and calmness. 
  • Tai Chi Flow for Health - Try tai chi and qigong exercises with a focus on gentle, low-impact mind-body healing. Modifications offered.
  • Yoga - Yoga will not be offered weekly in 2024, but past recordings are available.

Virtual Cancer Support Groups

Cancer Support Community

Our virtual support sessions allow you to safely access the support you need from the comfort of your home. Brought to you in partnership with Cancer Support Community. To attend, call 317-257-1505 for a brief interview, and then you will receive a virtual link for the group. Meet some of our group facilitators.

  • Prostate Support Group: 4th Wednesday @ 6:30 p.m.
  • Patient Support Group: 2nd and 4th Wednesday @ 6 p.m.
  • Living with Metastatic Cancer: 2nd Thursday @ 6:30 to 8 p.m.

Support Groups and Schedules

Young Adult Social and Support Group

Group of young adultsYoung adult cancer patients (ages 20-39) find a strong sense of belonging through Community's Young Adult Social and Support group (YASS). Join members monthly and be surrounded by love, support, and friendship to help you along your journey.
