Diabetes Care Center
The Diabetes Care Center of Community Health Network provides individualized, quality education and disease management programs and services to all people with diabetes. Our staff of nurse practitioners, nurses and dietitians provides short-term and long-term support, in collaboration with the patient's primary care physician or specialist to ensure that patients are appropriately managing their diabetes.
