A signed patient authorization is required by both state and federal laws in order to release a copy of your medical record or a copy of your itemized billing statement. Under these laws, a medical record and an itemized billing statement are considered protected health information and, as such, cannot be released without the authorization of the patient or the patient's legal representative. If you would like a copy of your medical record or a copy of your itemized statement, please print this form, complete it and mail it to the Health Information Department of Community Howard Regional Health, 3500 South Lafountain, Kokomo, Indiana 46902.
