Our Touchpoint Senior Living practice consists of experienced physicians and nurse practitioners, dedicated to serving the geriatric population in post-acute and long-term care settings. The physician-led provider team works closely with the care teams at partner skilled nursing facilities and various senior living care settings to achieve best possible outcomes for elderly patients.

Tammy Durham-Boring, D.O.
Dr. Tammy Durham-Boring earned her undergraduate degree in biological sciences from Washington and Jefferson College in Washington, Pennsylvania. She then earned her medical degree at the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa. She completed her internship and residency training in internal medicine at Des Moines General Hospital in Des Moines, Iowa. During her time there she was chief resident in internal medicine in her last year of residency and earned an excellence in internal medicine honorable distinction award.
Dr. Durham-Boring is board certified in internal medicine as well as hospice and palliative care medicine. Before joining Touchpoint Senior Living she had an inpatient hospital and outpatient ambulatory practice in internal medicine at Community Westview Hospital for 14 years. She is involved in caring for the aged population and has been a provider at multiple extended care facilities in Indiana for more than 12 years. She is also a certified nursing home medical director through AMDA. She is passionate about providing care for those with terminal or progressive chronic ailments.
Dr. Durham-Boring is married and has a daughter and multiple step-children. She enjoys reading, cross-stitching and spending time with family and friends.
Syeda Naqvi, M.D.
Syeda Naqvi, M.D., completed her medical degree at Sind Medical College in Karachi Pakistan and her residency training with the University of Kentucky Family Medicine program in Hazard, Kentucky. While working with the Eastern Kentucky Veterans Center, her interest in the elderly was enhanced, and she then specialized with a fellowship in Geriatrics from the University of Louisville. A highlight of her tenure there was receiving the prestigious Gold Star Employee Award for her sincere efforts and dedication to patients, families and primary care providers.
Dr. Naqvi believes that practicing medicine is all about compassion—serving humanity in the most intimate way possible. Through addressing seniors’ complex medical, physical and psychosocial needs, she works alongside the primary care provider to keep patients active, functional and in their homes as long as possible.
Dr. Naqvi splits her time between seeing patients at the Community Hospital East HATS clinic and attending on patients as part of the Touchpoint Senior Living team.
Having recently moved to Indiana to be closer to her family, Dr. Naqvi resides in Brownsburg. She enjoys spending time with family, including her 12-year-old son, decorating her house and cooking.
John D. Riley, M.D., MPH, CMD
Dr. Riley earned his medical degree at the University of South Florida, served his residency in internal medicine at East Carolina University and completed a Fellowship at Indiana University in general medicine. Dr. Riley has been recognized as a Certified Medical Director in Long Term Care by the Board of Directors of the American Medical Directors Certification Program.
He has spent the last 10 years in a house call and long-term care driven medical practice and is a certified home health care physician and a board-certified internal medicine physician. Dr. Riley enjoys working with geriatric populations, and his special interests include dementia, hospice and palliative care. He looks forward to bringing his knowledge of health care in the residential and long-term care settings to Touchpoint Senior Living and the patients it serves on the north side of Indianapolis.
Dr. Riley lives in Fishers with his wife, Dr. Treep, and his two sons.
Megan Linden, FNP
Megan Linden is a board certified family nurse practitioner through the American Association of Nurse Practitioners. She is licensed by the state of Indiana to provide care to individuals of all ages. She received her bachelor’s degree in the science of nursing from Indiana University Purdue University of Indianapolis, and her master’s degree in the science of nursing from Indiana Wesleyan University. Prior to joining the Touchpoint team, Megan worked for Community Home Health and in a long term care facility as a registered nurse where she discovered her love of geriatrics.
Megan is married to Maurice, and they reside in Indianapolis. She has one son along with multiple step children. Her hobbies include spending time outside, walking the family dog, and spending time with friends and family.
Sarah Minges, MSW
Sarah is a master’s level social worker who earned her bachelor of arts in psychology from Indiana University and later received her masters of social work through IUPUI in 2006. Sarah began her career working in international adoption serving as a case worker to assist families throughout the complexities of adoption process.
Sarah joined Community Health Network in 2005 as an inpatient case manager at Community Hospital North. Eight years later, she joined the Touchpoint Senior Living team in 2013. Sarah is passionate about working with geriatrics and prides herself on her ability to advocate and support patients and families during a medical crisis.
Sarah and her husband, Will, live in Fishers with their son and daughter. She enjoys spending as much time as possible with her two children, cooking, and spending weekend summers at her family’s lake cabin.
Mercy Moto, APN, FNP
Mercy is a board-certified family nurse practitioner through the American Nurses Credentialing Center (ANCC), a subsidiary of the American Nurses Association (ANA). She is licensed by the state of Indiana to provide care to individuals of all ages. She began her nursing career by receiving her associate in nursing degree from Midway College in Kentucky, then receiving her bachelors of nursing at Governors State University in Illinois. She then attended Indiana State University for her master’s in nursing. She obtained her family nurse practitioner license in 2012. Most of her nursing background has been in cardiac care and critical care. Prior to joining Community Touchpoint in 2016, she worked in an outpatient Federally Qualified Health Center (FQHC) in New Jersey and for CVS Minute Clinics.
In her spare time, she enjoys traveling with her husband, who is also a nurse practitioner. Together, they have three children. They originally migrated from Zimbabwe in 1994 and some of their greatest achievements include traveling to Zimbabwe to help with small projects in local schools.
Virginia E. Spears, MSN, FNP-C
Virginia (Ginny) Spears earned her masters of science in nursing from Indiana University and has been a nurse practitioner for the last 12 years, joining Community Health Network in 2006. She works with Touchpoint Senior Living patients on the east side of Indianapolis. During her 28 years in nursing she has worked in many areas, including surgery, open heart recovery, post-anesthesia care, peri-operative care, emergency nursing, dialysis, intensive care, critical care, psyche and in-patient detox. Education has long been a focus for Spears who developed a diabetic education program for St. Vincent Heart Center and has taught numerous classes for Ivy Tech Community College of Indiana and Indiana University. She is a member of Sigma Theta Tau, the American Academy of Nurse Practitioners and numerous other professional organizations. She is certified to teach Basic Life Support.
Spears and her husband Bob have been married for more than 40 years and have two grown sons. She enjoys spending time with family and friends, trips to the beach, and hiking, golf and tennis.
Brandi Wash, FNP
Brandi Wash is a certified family nurse practitioner and is licensed by the state of Indiana to provide family-centered healthcare to patients of all ages. Brandi’s educational background includes a bachelor’s degree and a master’s degree from Indiana Wesleyan University. Brandi joined Touchpoint in April 2012. Before completing her nurse practitioner degree, she worked in the hospital and the long-term care setting.
Brandi Wash is married to her husband Levy and she has three young girls. In her free time she enjoys traveling and watching sports and spending time with her family and friends. She is excited to be a member of the Community team and loves working for Touchpoint and taking care of people in the extended care facilities.
Kelly Wilson, MSW, LSW
Kelly is a master’s level social worker who earned her bachelor’s degree in psychology from Ball State University. She received her masters of social work through Indiana University in 2013. Kelly started her career as a psychiatric social worker at Community Hospital North where she enjoyed working with geriatric patients on the Seasons behavioral care unit. Kelly joined Touchpoint Senior Living in 2015 to expand her career and passion for working with older adults.
Kelly lives in Fishers with her family. She enjoys traveling, walking the dog, music, and spending time with family and friends.
