Through advanced technology and treatment, skilled and caring staff and physicians, and coordination of care across sites, Rehabilitation Services offers convenient and timely access with excellent outcomes.
Community Health Network Rehabilitation Services provides a comprehensive continuum of care throughout the rehabilitation process. Services include outpatient rehabilitation and sports medicine, inpatient rehabilitation, neurological care, stroke and brain injury services, and industrial rehabilitation. These services are offered as appropriate through five Rehab and Sports Medicine Centers, Hook Rehabilitation Outpatient Services, Inpatient Acute Units, and the Hook Rehabilitation Subacute Unit.
Community Rehab and Sports Medicine Centers have physical and occupational therapists with expertise to treat people of all ages. Physical or occupational therapy may be right for you if you have the following:
- Joint pain (hip, knee, shoulder, elbow, foot, or hand)
- Lower back pain
- Neck pain
- Pain after surgery
- Pain with pregnancy
- Problems with balance or walking
- Lymphedema and swelling problems
- Urinary or bowel incontinence
- Pelvic pain
Hook Rehabilitation Services provides traditional physical, occupational and speech therapy to individuals who are affected by stroke, multiple sclerosis, Parkinson’s disease, balance disorders, vestibular problems and communication difficulties. The Hook Rehab also team provides prosthetic and orthotic training and driving evaluations. |
