Through advanced technology and treatment, skilled and caring staff and physicians, and coordination of care across sites, we offer a comprehensive continuum of care with excellent outcomes.

Community Rehab & Sports Medicine Centers have physical and occupational therapists with expertise to treat people of all ages. Services include outpatient physical and occupational therapy, speech language pathology, pediatric services and more.

Community Rehab & Sports Medicine receives FOTO® Outcomes Excellence Certificate

December 21, 2010

Community Health Network’s rehab & sports medicine program has been awarded an Outcomes Excellence Certificate from Focus On Therapeutic Outcomes Inc. (FOTO®), a nationwide outcomes database and reporting service for health care providers. The certificates are presented to a select group of providers across the country to recognize the excellence achieved in effectiveness of treatment during the past 12 months. Community has demonstrated a greater increase in function for its patients than the national aggregate average. “In a health care system that is moving toward being outcome-driven, we feel we are ahead of the game,” said Nichole Wilson, director of rehab services for Community Health Network. “Our dedication to outcomes allows our patients to receive treatment that has documented effectiveness and efficiency.
