Community Health Pavilions

Community Health Network offers multiple health pavilions throughout central Indiana that provide convenient access to a wide range of health care services under one roof, including primary care and specialty physicians, diagnostic testing, rehab and sports medicine, and behavioral health services. Pavilion locations include Carmel, County Line Road (Greenwood), East Indianapolis, Fishers, Fort Harrison, Greenfield, Noblesville/Hamilton Healthcare Campus, Saxony/Olio Road, Shadeland Avenue and Washington Street.

Several of our pavilions feature a medical mall concept—one-stop shopping that allows patients to enter the building and access the practices and services they need from a common inside area. Our focus is allowing patients and families to meet their medical needs conveniently and efficiently, so they get in, get out and get back to their busy lives.

Community Health Pavilion Washington

On May 7, 2012, Community Health Pavilion Washington opened to serve Indianapolis east side residents. Services include urgent care, rehab and sports medicine, imaging, laboratory, and Community Physician Network practices. Learn more >>

Stones Crossing Health Pavilion

A groundbreaking ceremony for the Stones Crossing Health Pavilion in Bargersville, Indiana took place on April 11, 2012. The new health pavilion is slated to open in 2013. Learn more >>

An ABI screening test is a quick, painless and accurate test that will detect symptoms of peripheral arterial disease (PAD). Good screening candidates are those over age 45 with other risk factors for vascular disease, including diabetes, smoking, hypertension, abnormal cholesterol profiles, and/or family history.
