2009 Nursing Excellence Awards

The Community Health Network registered nurse and licensed practical nurse strive for excellence in professional practice. And now is your chance to nominate one (or more) for a 2009 Nursing Excellence Award.

Nomination information

2008 Nursing Excellence Award winners

Nominations will only be accepted online. The nomination process begins Wednesday, July 1, 2009 at 12:00 a.m. and runs through Friday, July 31, 2009 at 11:59 p.m.

One Nursing Excellence Award will be awarded in each of the five categories of Community nursing excellence:

  • Patient-focused care - (example)
    The nurse’s practice personifies patient-focused care. The nurse’s practice is aligned with Community Health Network values, demonstrates integrity in professional practice and ethical decision-making, is a role model to others in the ART of health care. The nurse’s practice is customized to the diversity, individuality and expectations of the people served.
  • Therapeutic relationships - (example)
    The hallmark of the nurse’s practice is therapeutic relationships. It demonstrates exceptional relationship competencies and an understanding of group dynamics.
  • Art and science of nursing - (example)
    The nurse’s practice reflects care proficient in the art and science of nursing as evidenced by: positive outcomes – patient, client, organizational, professional – embracing organizational and specialty standards of practice, recognition by nurse peers and at the community/national levels for expertise in nursing practice, certification in a nursing specialty when available and membership in professional nursing organizations.
  • Evidence-based practice - (example)
    The nurse’s practice is state-of-the-art, evidence-based and continuously improving. It validates an involvement in continuous self-learning activities and a commitment to scholarly opportunities.
  • Facilitative leadership - (example)
    The registered nurse’s practice demonstrates facilitative leadership in groups and teams. The nurse actively enhances the continuous learning and development of others through mentoring, precepting and role modeling.

Nominations will be anonymously reviewed and recipients will be selected by the Nursing Excellence Awards committee. The group will announce the names of winners and nominees for department celebrations in the fall. The awards will be presented at a banquet to be held on Friday, November 13, 2009 at the Indianapolis Marriott North. (map)

Eligibility

  • A nurse may be nominated by anyone—patient, physician, family member, co-worker or self.
  • A nurse may be nominated in multiple categories. Each nomination must focus on only one category.
  • A nominee must have worked as an R.N. or L.P.N. for Community Health Network for at least five years and must be in good standing without disciplinary issues.
  • Please provide an e-mail address to receive proof of receipt.
  • Please fill in all items in a category for a complete nomination.

All nominations must be received by 11:59 p.m., Friday, July 31 — NO EXCEPTIONS.
