Community Physician Network welcomes new providers

Indianapolis, IN--- Community Physician Network, the integrated, multi-specialty physician group at Community Health Network, welcomes the following providers:

Hari Kumar, MD, is a vascular surgeon who has interest in both open and minimally-invasive treatment of diseases of the aorta, carotid and extremity arteries. He also has interest in diseases of the venous system, thoracic outlet and dialysis access. Born and raised in Michigan, Dr. Kumar graduated from the University of Michigan with honors. He received his medical degree at University of Cincinnati College of Medicine and his general surgery residency at Indiana University School of Medicine. He completed his vascular surgery fellowship at Northwestern Memorial Hospital in Chicago. Dr. Kumar has two office locations: 1400 North Ritter Avenue, Suite 500, and 1402 East County Line Road, Suite 2400.

Navneet Lather, MD, is a cardiologist primarily focusing on the treatment of patients with heart failure. He attended Kasturba Medical College in Manipal, India, and West Virginia School of Medicine in Morgantown, West Virginia. He later completed his internship and residency in internal medicine at Columbia University Harlem Hospital Center in New York. He was chief internal medicine resident at Mount Sinai School of Medicine in Jersey City, New Jersey. He is board-certified in internal medicine, nuclear cardiology and echocardiography. He completed a heart failure and cardiac transplant fellowship as well as a cardiovascular disease fellowship at the University of Cincinnati College of Medicine in Cincinnati, Ohio. Dr. Lather is board certified in internal medicine, nuclear cardiology and echocardiography. He has three office locations: 8075 North Shadeland Avenue, Suite 200, Indianapolis; 1400 North Ritter Avenue, Suite 500, Indianapolis; and 1402 East County Line Road, Suite 2400, Indianapolis.

Jose Moreno, MD, is a general surgeon who was born in Bloomington and holds dual citizenship in the United States and Venzuela. He commenced his general surgery internship at Indiana University and completed his general surgery residency at Albert Einstein Medical Center in Philadelphia. After residency, he went on to receive fellowship training in Texas. Dr. Moreno’s primary interests are in the fields of minimally-invasive, laparoscopic and open abdominal surgery. He also treats common anorectal disorders including hemorrhoids, fissures and abscesses. He is fluent in English and Spanish. Dr. Moreno’s office is located at 1400 North Ritter Avenue, Suite 330, Indianapolis.

Sakkapol “Zach” Ongwijitwat, MD, is a board-eligible orthopedic surgeon who is fellowship trained in joint replacement and joint preservation. A graduate of the Medical College of Wisconsin, Dr. Ongwijitwat completed his internship and residency in orthopedic surgery at the University of California San Diego Medical Center. He then completed his fellowship at Washington University Orthopedics in St. Louis, Missouri. His professional interest is in utilizing evidence-based practice to reduce pain, shorten recovery time and minimize complication from joint replacement. Dr. Ongwijitwat has two Indianapolis office locations: 7930 North Shadeland Avenue and 1400 North Ritter Avenue, Suite 510.

Meredith Potrzebowski, DO, is a family medicine physician who was born in Anderson, Indiana, and raised in nearby Marion. After completing her bachelor’s degree in health sciences at Purdue University, she obtained her doctor of osteopathy degree at Lincoln Memorial University-Debusk College of Osteopathic Medicine. Dr. Potrzebowski completed her family medicine residency at Community Westview Hospital in Indianapolis, where she also served as chief resident. She enjoys seeing patients of all ages and has a special interest in keeping her patients well and living an active, healthy life. She also enjoys doing osteopathic manipulative treatment and acupuncture for various ailments including musculoskeletal pain. Dr. Potrzebowski’s office is located at 1675 North Main Street, Lapel.

Kristen Simpkins, MD, is a pediatrician who completed her medical degree at Chicago Medical School and her pediatrics residency at Lutheran General Children’s Hospital in Park Ridge, Illinois. She studied heavily in the area of adolescent medicine and has a special interest in taking care of teenage patients, although she enjoys caring for children of all ages. Dr. Simpkins’ office is located at 13121 Olio Road, Suite 340, Fishers.
