Community Physician Network welcomes new providers

Indianapolis, IN--- Community Physician Network, the integrated, multi-specialty physician group at Community Health Network, welcomes the following providers:

Anthony Arata, MD, is a family medicine physician who sees a variety of patients from pediatrics to geriatrics. Dr. Arata grew up in northern Indiana and completed his bachelor’s degree at the University of Notre Dame. He completed his medical degree at Saba University School of Medicine in Netherlands-Antilles and his family medicine residency at Indiana University. Dr. Arata’s office is located at 9560 E. 59th Street, Indianapolis.

Jayender Chintaparthi, MD, is an endocrinologist caring for patients on the south side of Indianapolis. Born and raised in India, he completed his bachelor of medicine and bachelor of surgery degrees at Kurnool Medical College in Kurnool, India. He obtained his Membership of the Royal College of Physicians (MRCP) in the United Kingdom. Dr. Chintaparthi then completed an internal medicine residency at Pennsylvania Hospital, University of Pennsylvania Health System and a fellowship in endocrinology, metabolism and diabetes at the University of Iowa. Dr. Chintaparthi’s office is located at 6920 S. East Street, Suite B, Indianapolis.

Alison Grant, MD, is a board-certified family medicine physician who completed her medical degree at Jefferson Medical College in Philadelphia, PA, and her family medicine residency at the Crozer-Keystone Health System in Chester, PA. Her office is located at 333 E. County Line Road, Suite B, Greenwood.

Dan Kraft, MD, is a board-certified sports medicine physicianspecializing in non-operative care of musculoskeletal and sports medicine injuries in pediatric and adolescent patients, including a strong focus on concussion management. Dr. Kraft completed his medical degree at the University of Illinois School of Medicine and his pediatric residency at Riley Hospital, Indiana University. He then completed a fellowship in sports medicine at Methodist Sports Medicine Center in Indianapolis. He has practiced pediatric and adolescent sports medicine in Indianapolis for more than 20 years and is co-director of the Indiana Sports Concussion Network. He has had direct involvement with developing recent Indiana state legislation relating to concussion care in high school athletes and serves as a team physician for Warren Central High School, Westfield High School and professional teams Indy Fuel and Indy Eleven. Dr. Kraft’s office is located at 8040 Clearvista Parkway, Suite 440, Indianapolis.

Daniel Livingston, MD, is a primary and critical care nocturnist serving Community Hospitals East and North. He was born and raised in Indianapolis and attended Avon High School. After completing his bachelor’s degree in biology at Butler University, he moved to Hungary to attend the University of Pécs Faculty of Medicine. He worked briefly in Cork, Ireland, before returning to Indianapolis to complete his internal medicine residency at St. Vincent Hospital. Dr. Livingston’s offices are located at 7120 Clearvista Drive, Suite 2100, Indianapolis, and 1400 N. Ritter Avenue, Suite 375, Indianapolis.

Reddy Pallavala, MD, is a pediatrician who received her medical training at Sri Ventakeswara Medical College in Tirupati, India, and then worked in the United Kingdom as a senior house officer in pediatrics. She moved to the U.S. and completed her internship at University Hospital in Brooklyn, NY, and her pediatric residency at Iowa Children’s Hospital in Iowa City, IA. Dr. Pallavala’s office is located at 3000 S. State Road 135, Suite 200, Greenwood.
