It can be difficult to remember the details of your medical history like insurance, medications and past medical procedures.

Now you can log all your information and reference it quickly in one convenient, secure place with the myCommunity Personal Health Record (PHR).

Maintain and update your records using the online medication and allergy guides. Use the online health library to identify topics that relate to you. Enter your cholesterol level, blood pressure, and other health measures from your most recent physical exam. You can do all of this and more to keep track of your general wellness. You can even print all your information to share with family members or your physicians.

Simply put, the PHR is a way for you to become more actively engaged in your health care and keep it coordinated in one place. It can also serve as a valuable source of information for you, your family and your doctors in times of emergency or concern.

At this time, your online Personal Health Record is an individualized account of your own medical history. Therefore, you cannot add family members to your PHR. However, as this technology continues to develop, you will be able to add children under the age of 18 to your PHR.
