Create a Personal Health Record

It can be difficult to remember the details of your medical history like insurance, medications and past medical procedures.

Now you can log all your information and reference it quickly in one convenient, secure place with the myCommunity Personal Health Record (PHR).

Personal Health RecordMaintain and update your records using the online medication and allergy guides. Use the online health library to identify topics that relate to you. Enter your cholesterol level, blood pressure, and other health measures from your most recent physical exam. You can do all of this and more to keep track of your general wellness. You can even print all your information to share with family members or your physicians.

As you develop your PHR, you will find that it's a great source of information, and it's easy to maintain. You simply add information as needed for a comprehensive snapshot of your medical history.

At this time, your online Personal Health Record is an individualized account of your own medical history. Therefore, you cannot add family members to your PHR. However, as this technology continues to develop, you will be able to add children under the age of 18 to your PHR.
