Community Health Network is headquartered in Indianapolis and is composed of seven corporate entities, both not-for-profit and for-profit. It also comprises five hospitals, including a dedicated heart hospital; 70 primary care physician practices; five free-standing Indiana Surgery Centers; five MedCheck urgent care centers; three MEDPOINT express clinics; seven occupational health locations; five Community Health Pavilions; nursing homes; and other health care facilities.

Community Hospital Anderson
Community Hospital Anderson serves the medical, health and human service needs of the people in Anderson, Madison County and contiguous counties with compassion, dignity, respect and excellence. Service, although focused on injury, illness and disease also embraces prevention, education and alternative systems of health care delivery.

The Indiana Heart Hospital
We’re all heart. At The Indiana Heart Hospital, we have raised the standard for cardiovascular care in our community, and the nation. Designed by cardiac doctors and nurses, our facility and approach to cardiac care are models for other facilities around the country. Our innovative services include the only 24-hour emergency room for heart care in Indiana, a cardiologist always on site for emergencies and advanced digital technology, which assures faster and more effective care.
Community Physicians of Indiana
Community Physicians of Indiana is more than 145 physicians at over 60 practices working in partnership with Community Health Network. We’re dedicated to providing remarkably convenient, patient-friendly care in areas including Family Practice, Internal Medicine, OB/GYN and Pediatrics.
Indiana Surgery Center
In considering outpatient surgery, one of the most important decisions a patient and a surgeon make is the choice of facility where that surgery is performed. When stepping into the Indiana Surgery Center, you enter an environment created especially for the patient. Every staff member, every process, every room in the building exists to respond to patient needs. The goal of the staff at the Indiana Surgery Center is for every patient to see the Center as an extension of the surgeon’s practice, representing the highest standard of care.
MedCheck
With five locations throughout central Indiana, MedCheck’s board-certified physicians and registered nurses can treat a variety of adults’ and children’s injuries and illnesses, plus provide many tests, therapies and screenings.

Community Health Pavilion
This new health care concept houses a diverse cross-section of health care services-from a MedCheck immediate care center to primary care physicians, from cardiology to pediatrics, a diagnostics testing center and a sleep lab. Hook Rehabilitation Center
Hook Rehabilitation Center is a comprehensive rehabilitation program that includes inpatient and outpatient services dedicated to the needs of the physically disabled and the cognitively impaired. Hook Rehab assists individuals to become more independent and return to their role in the family and community of which they are a part.

Indianapolis Endoscopy Center
The Indianapolis Endoscopy Center is a licensed surgery center dedicated to the diagnosis, treatment and follow-up of gastrointestinal disease. Founded in 1994, Indianapolis Endoscopy Center is committed to providing quality care to all patients. We strive to maintain and improve our services through a continual quality improvement program.
Community Imaging Center
Community Imaging Center is a comprehensive outpatient imaging center. We offer early morning, evening and even weekend appointments. Patients of all ages are welcome, from teenagers to elderly individuals.
Diabetes Care Center
Through education, nursing assessments and a multidisciplinary team, the Diabetes Care Center staff help patients better manage their disease and insulin therapy. This type of improved control can improve patients' quality of life and reduce the risk of serious complications as well as the high cost of caring for those complications. Diabetes Care Center services provide support to the physician caring for patients with diabetes.
Wellspring Pharmacy
Wellspring is a unique pharmacy that specializes in whole health. We not only cater to patients with disease states but we also focus on promoting overall health and wellness. With the help of our knowledgeable pharmacists, on-site nurse educator and caring staff, we help families and individuals reach health goals at every stage of life. This philosophy is the foundation from which we have built a truly patient-centered pharmacy.
