The Indiana Heart Hospital
We’re all heart. At The Indiana Heart Hospital, we have raised the standard for cardiovascular care in our community, and the nation. Designed by cardiac doctors and nurses, our facility and approach to cardiac care are models for other facilities around the country. Our innovative services include the only 24-hour emergency room for heart care in Indiana, a cardiologist always on site for emergencies and advanced digital technology, which assures faster and more effective care.

Community Physicians of Indiana
Community Physicians of Indiana (CPI), one of the area’s largest primary care medical groups, includes more than 200 primary care physicians and nurse practitioners in more than 70 locations across central Indiana. Center for Wound Healing
Every wound has its unique set of circumstances. Our Center for Wound Healing is equipped and staffed to address them all. We are experts at caring for people whose open sores have resisted traditional treatment, with a success rate of 80% healed in 12-16 weeks. Community Health Pavilions
This new health care concept houses a diverse cross-section of health care services-from a MedCheck immediate care center to primary care physicians, from cardiology to pediatrics, a diagnostics testing center and a sleep lab. The idea is to meet the majority of an individual’s or a family’s primary health care needs under one roof, creating a one-stop approach to health care.

Community Imaging Center
Community Imaging Center is a comprehensive outpatient imaging center in Indianapolis offering the latest in radiology and imaging technology. Common procedures include MRI, ultrasound, Lightspeed VCT, CT scan, X-ray and voiding cystourethrogram (VCUG). We offer early morning, evening and even weekend appointments. Patients of all ages are welcome, from children to elderly individuals.

Community Spine Center
Community Spine Center, part of Community Health Network in Indianapolis, Indiana, provides state-of-the-art care for patients with spinal pain or dysfunction.

Diabetes Care Center
The Diabetes Care Center of Community Health Network provides individualized, quality education and disease management programs and services to all people with diabetes. Our staff of nurse practitioners, nurses and dietitians provides short-term and long-term support, in collaboration with the patient's primary care physician or specialist to ensure that patients are appropriately managing their diabetes.

Indiana Surgery Center
In considering outpatient surgery, one of the most important decisions a patient and a surgeon make is the choice of facility where that surgery is performed. The Indianapolis Endoscopy Center is a licensed surgery center dedicated to the diagnosis, treatment and follow-up of gastrointestinal disease. Founded in 1994, Indianapolis Endoscopy Center is committed to providing quality care to all patients. We strive to maintain and improve our services through a continual quality improvement program.

MedCheck
With five locations throughout central Indiana, MedCheck’s board-certified physicians and registered nurses can treat a variety of adults’ and children’s injuries and illnesses, plus provide many tests, therapies and screenings.

Many of Community's shops are operated in partnership with the Community Health Network Foundation, with proceeds directly supporting the mission of Community Health Network. Community gift cards are accepted at all Community Health Network shops.
