Outpatient services for serious mental illness

Specialized services are targeted to people who suffer from schizophrenia or other psychotic or mood disorders that significantly impair their ability to function. Services focus on medication education and management, activities of daily living, clinical case management, clubhouse and ACT team, individual and group therapy as well as family support. Residential services work with therapeutic services to care for the individual and family.

All services for this population are coordinated and provided by teams of case managers working from a recovery philosophy and may include some or all of the following:

  • Assertive Case Management
  • Activity of Daily Living (ADL) training
  • Medication evaluation, education and management
  • Integrated treatment of mental health and chemical dependency problems
  • Individual therapy
  • Family therapy
  • Group therapy
  • Psychological testing
  • Forge Ahead Psychosocial Rehabilitation Clubhouse
  • Supported employment
  • Residential services (group homes and supervised apartment living)

Treatment staff

Multidisciplinary team—under the direction of psychiatrists—includes psychologists, advanced practice nurses, licensed clinical social workers, licensed master's-level therapists in counseling, and addictions-certified master's-level counselors.

Our Town

Our Town is a subgroup of the Assertive Case Management team that serves young adults, called members, between the ages of 18 and 25, who have been diagnosed with a severe and persistent mental illness. Staff is recovery focused, providing strengths-based treatment with the intent of assisting members in becoming self-sufficient. In addition to receiving education about their mental illness, members develop the skills they need to successfully navigate their world.
