  • What is hypoglycemia?–Hypoglycemia is a condition characterized by a glucose (blood sugar) level that is too low to effectively fuel the body's blood cells. Glucose is the main source of fuel for the body. Hypoglycemia may be a condition by itself, or may be a complication of diabetes or another disorder.   • Diabetes and eye care–Diabetic eye disease refers to a group of eye problems that may occur in persons with diabetes as a complication. All can cause severe vision loss or even blindness. Diabetic eye disease can often be treated before vision loss occurs. All people with diabetes should have a dilated eye examination at least once a year. About Diabetes Care Center

Diabetes Care CenterThe Diabetes Care Center of Community Health Network provides individualized, quality education and disease management programs and services to all people with diabetes. Our staff of nurse practitioners, nurses and dietitians provides short-term and long-term support, in collaboration with the patient's primary care physician or specialist. A behavioral psychologist is available to meet patients as needed to work on problems related to chronic illnesses, such as anxiety and depression. Our goal is to ensure that patients are appropriately managing their diabetes in between visits to their physician.

Patients referred to the Diabetes Care Center will be scheduled for a one-hour individual assessment, followed by individual or group appointments based on their needs. Offerings include educational sessions, insulin pump management, pregnancy monitoring, pediatric services, cardiac risk assessment and prevention, and general disease management.

The Diabetes Care Center also provides a transition for patients from the hospital to the outpatient setting. Inpatient diabetes care coordinators regularly monitor patients with elevated blood glucose and A1C levels and collaborate with multidisciplinary staff for care management. They coordinate referral into the Diabetes Care Center as a bridge between the inpatient stay and outpatient follow-up with a primary care physician or specialist. Nurse practitioners are also available for consultation for inpatients who have diabetes or who are experiencing stress-related hyperglycemia.

Physician referral is necessary for participation in all diabetes programs for insurance billing purposes. The Diabetes Care Center will work with patients based on their insurance coverage. Referral to appropriate social service agencies will be made for those patients who do not have insurance coverage or who have financial concerns.
