Saturday: 8 a.m. to 12 p.m.

To schedule an appointment, call 317-621-3646 or 866-621-3646 (toll free).

When you call, we will ask for your:

• Date of birth
• Social security number
• Insurance carrier and information from your insurance card
• Diagnosis
• Name of the test your doctor ordered
