Our Staff

Doug BeebeDoug Beebe
Executive Director

I have been involved in the fields of aging and disability since 1983. The main emphases have been developmental disabilities, traumatic brain injury and aging. I graduated from Wabash College in 1983 with a B.A. in psychology. I received an M.S. in psychology with an emphasis in rehabilitation from Purdue University in 1992. I am also licensed as a health facilities administrator in Indiana.

An area of special interest over the last several years has been systems change. Through my work with the state, I became very familiar with CICOA and its mission, and am currently on the Board of Directors for CICOA. I worked closely with the advocacy and provider communities to drive changes into Indiana’s systems, and became involved with the basic needs committee of the AdvantAge Initiative. I have maintained that interest and those working relationships. I currently serve on the Boards of Directors of both the Brain Injury Association of Indiana and the Indiana Association of Homes and Services for the Aging. These agencies have many common interests with CICOA, and it appears the work can be complementary. I also serve as a member on the Mayor's Advisory Council, which emphasizes inclusion and awareness of persons with disability in Indianapolis.

 

Physiatrists | What is a physiatrist?

Allison Williams, M.D.Allison Williams, M.D.
Medical Director

Dr. Williams is a rehabilitation physician and is board certified in the field of physical medicine and rehabilitation. She completed medical school and residency training at the Indiana University School of Medicine. Dr. Williams' areas of special interest include brain injury, stroke and electrodiagnosis.

Dr. Williams is active in the American Medical Association and the Indiana State Medical Association, and is currently president of the Indiana Society of Physical Medicine and Rehabilitation.

Dr. Williams joined Community Health Network in 2006. She is currently medical director of the Hook Rehabilitation Center's brain injury program.

Marc Duerden, M.D.Marc Duerden, M.D.

Dr. Duerden is a specialist in rehabilitation and is board certified in physical medicine and rehabilitation. He is also board certified in spinal cord injury medicine.

Dr. Duerden attended Indiana University School of Medicine and received specialty training in physical medicine and rehabilitation at Northwestern University. He is a clinical associate professor at Indiana University School of Medicine in the department of Physical Medicine and Rehabilitation. His academic responsibilities include teaching medical students and other physicians who are in training for the specialty in physical medicine and rehabilitation.

Dr. Duerden is active in the Indianapolis Medical Society and the Indiana State Medical Associations (ISMA). He is the past president of the Indianapolis Medical Society, Indiana Society of Physical Medicine and Rehabilitation and the Young Physicians section of ISMA. He has been selected to work with the national accreditation committee for Physical Medicine and Rehabilitation residency training programs.

Dr. Duerden believes strongly in the team concept for recovery, integrating all members of the team to enhance a patient’s function.

Anthony Mimms, M.D.Anthony Mimms, M.D.

Dr. Anthony Mimms is the medical director of the Hook Subacute Rehabilitation Unit. Dr. Mimms is an Indianapolis native. He received his bachelor’s degree in biology from Wabash College and his medical degree from Indiana University. He completed his residency in physical medicine and rehabilitation at William Beaumont College in Royal Oaks, Michigan.

Dr. Mimms is involved in several charitable organizations, including the Little Red Door and the Take Five Program.

Neuropsychologists

David Klein, Ph.D., H.S.P.P.David Klein, Ph.D., H.S.P.P.
Clinical Coordinator

Dr. David Klein completed his doctorate in psychology at Indiana University. His internship in neuropsychology and behavior medicine was completed at the Medical College of Ohio. He then completed a post-doctoral fellowship in neuropsychology through the Indiana University School of Medicine and Community Hospitals. Dr. Klein joined Community Health Network in 1995. He is also a clinical assistant professor in the Department of Physical Medicine and Rehabilitation at the Indiana University School of Medicine teaching physician residents theory, research findings, and clinical practice of neuropsychology. Past research areas include post-acute brain injury rehabilitation, neural control of movement and short-term memory in monkeys, and conceptual skills employed by therapists within the therapeutic session.

Dr. Klein has specialized in post-acute brain injury rehabilitation and currently serves as the Clinical Director of the Hook Rehabilitation Brain Injury Continuum. Responsibilities include maintaining program quality and integrity of the inpatient NeuroBehavioral Unit and post-acute Outpatient Day Treatment Brain Injury Program. Dr. Klein provides staff consultation and training in holistic brain injury rehabilitation techniques and participates in all relevant patient staffings to ensure high-quality care throughout the brain injury continuum.

Edmund C. Haskins, Ph.D., H.S.P.P.Edmund C. Haskins, Ph.D., H.S.P.P.

Dr. Edmund C. Haskins is a licensed psychologist and an Indiana state board-certified health service provider in psychology. He is a clinical neuropsychologist at Hook Rehabilitation Outpatient Services within Community Health Network. Dr. Haskins coordinates treatment services for outpatients with stroke and brain injury in the Day Treatment Program in the Hook Rehabilitation continuum. He also provides comprehensive neuropsychological and personality assessments as well as psychotherapy and marital therapy for individuals with brain injury.

Dr. Haskins received his Ph.D. in clinical psychology from the University of Florida in 1986 and completed a post-doctoral fellowship in clinical neuropsychology at the Veterans Administration Medical Center, Menlo Park, California. He is affiliated with the National Academy of Neuropsychology, the International Neuropsychological Society and the American Psychological Association.

Michael P. Shain, Ph.D., H.S.P.P.Michael P. Shain, Ph.D., H.S.P.P.

Dr. Michael Shain completed graduate studies at Illinois Institute of Technology and obtained his doctorate in psychology at Southern Illinois University. His internship in geropsychology and behavior medicine was completed at Rush-Presbyterian-St. Luke’s Medical Center. Dr. Shain joined Community Health Network in 1990. He is also an adjunct professor in the Psychology Department at IUPUI and the University of Indianapolis providing practicum supervision to psychology graduate students.

Dr. Shain has specialized in stroke/brain injury rehabilitation and currently serves as the neuropsychologist for the Hook acute brain injury and Hook neurobehavioral programs. Responsibilities include provision of psychological and neuropsychological services for patients and their families, participation in team staffings, and leadership for patient-family conferences to selected patients on those units. Dr. Shain also has strong interest in geriatric rehabilitation and provides staff consultation and psychological services to patients of the Hook subacute rehabilitation program. As part of his interest in stroke rehabilitation, Dr. Shain is the facilitator of the Hook stroke support group and peer visitor program.

Les Suga, Ph.D., H.S.P.P.Les Suga, Ph.D., H.S.P.P.

Dr. Suga completed his internship and post-doctoral fellowship in clinical neuropsychology at The Ohio State University College of Medicine prior to coming to the Community Health Network in 1988. During his time at OSU he was involved in research into the use of many of the neuropsychological instruments that are used at our rehabilitation center, their clinical properties, and their use in different clinical populations. During his time at the Hook Rehabilitation Center he has served in many different capacities in our brain injury program, our epilepsy surgery program, and our outpatient clinic in neurosurgery services at Wishard Memorial Hospital and currently is the neuropsychologist in our stroke and spinal cord services.

Within the network, Dr. Suga is the chairman of the Psychology Division at Community Hospital East, Community Hospital North and The Indiana Heart Hospital. He also is the continuing education coordinator for psychologists in the network. He serves as the contact person between the network and the Indiana State Board of Psychology and continues to work to keep Community Health Network an accredited provider of state-approved psychology courses for licensure requirements.

Daniel J. Venezia, Jr., Ph.D., H.S.P.P.Daniel J. Venezia, Jr., Ph.D., H.S.P.P.

Dr. Venezia is a clinical neuropsychologist in Hook Rehab Neuropsychological and Psychological Diagnostic Services. He is licensed as a psychologist in the state of Indiana, and he holds a Diplomate in clinical neuropsychology from the American Board of Professional Psychology. Dr. Venezia’s practice primarily involves the neuropsychological evaluation of adults and children with known or suspected, acquired or developmental, brain dysfunction.

Dr. Venezia received his Ph.D. in clinical psychology from Washington University, St. Louis, Missouri, in 1972. From 1981-1982 he completed a post-doctoral fellowship in clinical neuropsychology at the Madigan Army Medical Center, Tacoma, Washington. He is a member of the National Academy of Neuropsychology.

Karen MayKaren May, Brain Injury Continuum Liaison

Karen May has been a nurse for 40 years and has worked for Community Hospital East for 32 of those years. Karen is responsible for the coordination of services for the person with brain injury/stroke across our continuum of care. She is a certified rehabilitation nurse and is a Certified Brain Injury Specialist (CBIAS). Karen is a lifetime member and served as chairperson of the Brain Injury Association of Indiana (BIAI) and its Board of Directors.

At the national level, Karen has served on the Board of Directors of the Brain Injury Association of the United States (BIAUSA). In addition, she has served as a member of various organizations such as Mayor’s Advisory Council, Statewide Needs Assessment Task Force of Persons with Brain Injury, TBI Planning Grant Committee, and Ohio Valley Advisory Council for Prevention and Treatment of Brain Injury. Karen has testified before the state legislature on several occasions for creation and passage of the TBI waiver, and Medicaid reimbursement issues. She was instrumental in designating the term “brain injury” for official use in public policy statements. Karen assists with the education of clients, families, external sources and professional organizations. She has devoted her career to the support and advocacy of patients and families.
