Hook Rehabilitation Center

Established in 1974, Hook Rehabilitation Center was the first inpatient rehabilitation center in Indiana. Hook Rehabilitation Center is a comprehensive program dedicated to providing for the rehabilitation needs of individuals who sustain physical disabilities or cognitive disabilities. We help people achieve their highest level of success and support them in returning to their family and community.

Click here to read Hook Rehab patient success stories.The Commission on Accreditation of Rehabilitation Facilities (CARF) accredits Hook Rehabilitation Center for comprehensive rehabilitation in both inpatient and outpatient settings. Hook has also received special designation from CARF for the following rehab programs: brain injury rehab for adolescents and adults and stroke rehab for adolescents and adults.

Hook Rehab once again achieved the highest level of CARF accreditation in May 2009. All of its rehab programs met the stringent requirements necessary to receive this designation of superior quality of care.
