Hook Rehabilitation Center
Established in 1974, Hook Rehabilitation Center was the first inpatient rehabilitation center in Indiana. The Commission on Accreditation of Rehabilitation Facilities (CARF) accredits Hook Rehabilitation Center for comprehensive rehabilitation in both inpatient and outpatient settings. Hook has also received special designation from CARF for the following rehab programs: brain injury rehab for adolescents and adults and stroke rehab for adolescents and adults.
Hook Rehabilitation Center is an 84-bed comprehensive program dedicated to providing for the rehabilitation needs of individuals who sustain physical disabilities and/or cognitive disabilities. We help people achieve their highest level of success and support them in returning to their family and community.
