Home Care

Community Home Health Services allows patients to remain safely and comfortably in the comfort of their home. Our staff works closely together to provide skilled medical care, education, and support to the patient and caregivers so they can move toward independence in learning how to manage their medical condition. Home care services are provided under the direction of the patient’s physician. We always have a nurse available 24 hours a day, seven days a week to meet the needs of our patients.

For all of our services, the staff works very closely with each other and the patient’s physician to report progress and any changes in the patient’s condition. A patient’s care may include a range of treatments that are coordinated to promote good patient outcomes and patient satisfaction.
