Jessie Westlund, R.N.
CEO of Community Home Health Services

Jessie Westlund, R.N., received her nursing degree from New England Baptist Hospital School of Nursing. She has served as CEO of Community Home Health Services since 1998. Prior to that, she held the role of exective director for the Visiting Nurse Association of North Central Indiana, directing 50 percent growth in the home health care agency in less than two years. In 2006, Jessie won the "Norah J. McFarland" award for leadership, integrity, community service, sensitivity to client’s needs, action on the behalf of homecare, interaction with other providers and statesmanship from the Indiana Association for Home & Hospice Care, Inc.

Jessie is active in the community, serving on the Hutson School Board, the Indianapolis Discovery Network for Dementia Board, LPN Advisory Board, and is a member of the Society for Nursing Excellence. Jessie is also wife and mother. She is married to Daniel Westlund and they have three sons who are also married and live in the area. They also enjoy the new additions to their family, their grandchildren. Jessie holds family values very dear and works to maintain a healthy balance of career and family.

Dale Theobald, Ph.D., M.D.
Medical Director of Hospice
Director of Symptom Management Group

Dr. Dale Theobald is the medical director of Hospice Services for Community Home Health Services (CHHS). Dr. Theobald is certified in psychiatry and addictions. He also holds a doctorate degree in clinical psychology, with more than twenty years of experience in caring for patients with terminal illnesses. Dr. Theobald has published many articles on symptom assessment and management. In collaboration with Community Regional Cancer Care (CRCC) staff, he is currently researching the treatment of pain, depression and other cancer symptoms, including the electronic reporting of symptoms.

Dr. Theobald oversees all CHHS hospice patient care. Whether the patient is an inpatient within Community Health Network, in a private residence, an assisted living resident, or living in a contracted extended care facility, his holistic approach toward patient care helps improve the quality of life for our patients. Dr. Theobald hopes to reduce the barriers to hospice care through strengthening the partnership between Hospice staff, patients, their primary care physicians and their caregivers.

Monica Brunelle, M.D.
Medical Director for Community Health Network Life’s Journey Program

Monica Brunelle, M.D. received a bachelor of science degree in pharmacy from Purdue University in West Lafayette, Indiana and a master of science degree, also in pharmacy, from Butler University in Indianapolis, Indiana. She went on to obtain her medical degree from Indiana University School of Medicine, completing an internship at Methodist Hospital (Indianapolis, IN) and residency in internal medicine at St. Vincent's Hospital (Indianapolis, IN).

She has practiced internal medicine since 1976 and is board-certified in both internal medicine and geriatrics, and most recently in hospice and palliative medicine. She is a certified medical director by the AMDA as well as a member of the Indiana State Medical Association, American Medical Association, American Medical Directors Association, and National Hospice and Palliative Care Organization, among others. She currently serves as medical director for Community Health Network's Life's Journey program as well as several other health care organizations.
