Pre-orientation

Prior to your paperwork and health assessment appointment, please read the following. Completing the documents below in advance will expedite your appointments. If you have any specific questions, please contact your health care recruiter.

Pre-orientation paperwork

Your paperwork appointment will take place at:

You should bring with you:

• Driver’s license**
• Social Security card**
• Voided check or deposit slip
• License/certification for the position (if applicable)
• Diploma or official transcripts as instructed by your recruiter
• Completed tax forms (Federal W-4 | State WH-4)

You must bring your diploma or official transcripts in addition to your licensure or certification for the position.

** Please note: The last name on the driver’s license and Social Security card must match. If not, you will not be able to start until you provide HR with an updated driver’s license or Social Security card/request receipt (two pages). If you bring this receipt, HR also needs to make a copy of your birth certificate.

If you need to obtain a replacement Social Security card, name change, or if you do not have a Social Security number, you may visit the Social Security Administration online for more information.

• Driver’s license
• Completed Health History form
• Current prescription medications (in the bottles)
• Shot/immunization records (if available)
• Copy of recent PPD (if available)

Orientation

All new employees (with the exception of internal transfer employees) are required to attend the orientation.

Location

On the first day of your orientation, please report at 7:45 a.m. in the main lobby at Community Hospital East. You will then be guided to the location of orientation activities.
