Referrals

Physician referral is necessary for participation in all diabetes programs for insurance billing purposes. The Diabetes Care Center will work with patients based on their insurance coverage. Referral to appropriate social service agencies will be made for those patients who do not have insurance coverage or who have financial concerns.

Patient records are managed on Logician® for easy access by Community Health Network physicians. Physicians referring from outside the network receive next-day chart updates by fax, as appropriate.
