Dr. Thomas Cittadine was born and raised in Elkhart, Indiana. He received his bachelor’s degree from Indiana University and his doctor of medicine from Indiana University School of Medicine. He completed his orthopedic residency at Lutheran Hospital in Ft. Wayne, Indiana and his fellowship training in hand surgery at the Indiana Hand Center. He was also a flight surgeon for the United States Air Force.

Dr. Cittadine is a board certified orthopedic surgeon specializing in hand and sports medicine injuries. Although fellowship trained in hand surgery, Dr. Cittadine provides a variety of orthopedic non-surgical and surgical treatments including general orthopedics for pediatric and adult patients.

Clifford Evans, D.O.

Clifford J. Evans, D.O., is a board certified orthopedic surgeon. His practice focuses on the evaluation and treatment of afflictions of the bones, joints, muscles, tendons and nerves. He has a special interest in sports injuries and working with young athletes.

Dr. Evans is board certified by the American Board of Orthopaedic Surgery and a member of the American Academy of Orthopaedic Surgeons.

From 2008 until 2012, Dr. Evans was staff orthopedic surgeon at Blancfield Army Community Hospital, Fort Campbell, Kentucky. He joined the Army in 1988, and was a pilot in the 101st Airborne, serving in the Gulf War, Hurricane Andrew relief, and in Honduras. He was a flight operations officer in Italy, and a mission commander, instructor, and executive officer in the 160th Special Operation Aviation Regiment. George Feliciano, M.D.

Dr. Hayes completed his residency in orthopedic surgery at Western Michigan University Homer Stryker, M.D. School of Medicine in Kalamazoo, Michigan, and received his specialty fellowship training from Harvard University in Boston Massachusetts.

Dr. Hayes’ areas of expertise include complex foot and ankle reconstruction, total ankle replacement, total knee replacement, cartilage restoration, and joint preservation of the foot and ankle. He also performs minimally-invasive foot and ankle surgery, arthroscopy of the knee, foot and ankle, bunion correction, hammertoe correction, and treatment for sports injuries of the lower extremity.
