Welcome to Family Medicine Care, Broad Ripple! It is our philosophy that we partner with you in your medical care. Therefore, we have some important policies and expectations to share with you.
At your appointment, you may see one of our nurse practitioners instead of the doctor: Kelly Burrough, RNC, MSN, Mylinda Cripes, NP-C or Kathryn McGivern, NP-C. As clinical professionals, our nurse practitioners are qualified to address and treat any urgent medical needs you may have.
We will do our best to accommodate your scheduling needs. We do ask that you give us a 24-hour notice when canceling an appointment. It you fail to show up for more than three appointments, you may be dismissed from the practice.
To schedule online, you must be a myCommunity member. Registration is free and takes only a few minutes. Register now, or if you are already a member, log in to myCommunity to schedule your appointment.
A fully completed, current patient registration will be on file in your chart during all times you are considered an active patient. You will be asked to fill out a new registration form at the beginning of each year. We will confirm with you at each appointment that your information is the same. We will also ask to see your current insurance card at each visit. It is important that you inform our office of any name, address, phone number, or insurance changes as soon as they occur.
Our office will file a claim with your insurance upon submission of proof of insurance (i.e., insurance card indicating coverage, identification and group number). In the event you have insurance coverage but cannot provide documentation, payment is due at the time of service.
Patient financial responsibility
If you do not have insurance coverage, payment is due at the time of service. Co-payments and balances from prior services are due at the time of service before the start of your appointment. If you have an unpaid balance that you need to make payment arrangements for, you must contact our office at 317-621-3680. It is important that you call to make payment arrangements or we will assume that you do not intend to pay your balance. Your account could be placed in collections at that point. Children under the age of 18 will require the signature of a responsible adult party on the registration form. A parent of the minor must accompany the patient to their appointment.
Our physician does not discuss financial issues. Our office manager is trained to discuss your account and make payment arrangements.
