I signed up for a myCommunity account. Why haven’t I received my card yet?
Card packets are mailed within two to four weeks from the day you sign up as a myCommunity member. If you haven’t received your card in that time, please send us an e-mail or call 800-777-7775 for further assistance.
If I (or someone I know) don't have computer or Internet access, can I still be a myCommunity member?
Yes! Please call 800-777-7775 and a Community Health Network representative will register you by phone. Or, you can ask to receive a registration packet through U.S. mail.

How do I put money on a myCommunity card?
You may add money to a card at any Community Hospital gift shop. Ask the manager for assistance.
Do myCommunity cards come loaded with money?
No, myCommunity cards do not come loaded with money.
I don’t think I have a myCommunity account. Why did I receive a card?
You have registered as a myCommunity member at some time in the past, possibly with a different e-mail address from the one you currently use. All Community Health Network employees who completed a personal wellness profile in 2007 and 2008 registered for a myCommunity account. myCommunity cards are never sent in an unsolicited manner.
