For an appointment or cancellation, please call centralized scheduling at 317-355-4682. If you cannot keep your appointment, please call at least 48 hours prior to your scheduled appointment.

Pre-registration

Please pre-register prior to your test by calling 317-355-3920 or 877-217-7355. You may also pre-register online via our secure pre-registration system.
