Our care managers can direct you and your family to the appropriate resources following an in-home comprehensive assessment (recommended), or by quick reference telephone consultations. The in-home assessment includes a formal written report and a one-hour family case conference that can be coordinated by phone or in person at a Community Health Network location.

- Arrangements for home medical equipment and safety alert devices
- Arrangements for house cleaning, yard maintenance and pet care assistance

- Coordination of moving needs

Deb Bosler, R.N., CCM, is Community Touchpoint’s senior care manager. Deb has been a registered nurse for over 30 years and is a certified case manager through the Commission for Case Manager Certification®.
Deb has also received the distinction of becoming certified as a professional geriatric care manager through the National Association of Professional Geriatric Care Managers.
A professional geriatric care manager (GCM) is a health and human services specialist who helps families who are caring for older relatives. The GCM is trained and experienced in any of several fields related to care management, including nursing, gerontology, social work or psychology.
Deb has many years of experience in acute care, hospice and home health, and has worked in skilled nursing and rehabilitation centers as well. Deb has a passion for senior adults and has been working with seniors and their families in Indianapolis and the surrounding communities since 2000.
