ImPACT testing
Community Health Network is pleased to participate and support a concussion management program utilizing the ImPACT concussion evaluation system. The testing allows a baseline and post-concussion assessment to better manage concussion for athletes of all ages. Learn more >>

The following schools are hosting 2013-2014 sports physical clinics. You may download the IHSAA sports physical form to complete prior to arriving for your sports physical.
