Behavioral care at home
Persons experiencing mental health problems may benefit from having a specially trained behavioral care nurse to provide assistance in their home. The role of the behavioral care nurse is to provide education to the patient and caregiver on what to expect and how to cope with the patient’s mental health problems.
Some additional services that may be provided by our behavioral care nurse are:
- Education on and assessment of the effects of psychotropic medications
- Behavior management program
- Post ECT assessments
- Coordination with outpatient behavioral care therapy, if needed, for further support and treatment when the patient no longer needs, or is no longer eligible for, home care services
- Coordination of the plan of care as directed by the patient's physician
If you or your loved one could benefit from home behavioral care, please contact your physician or call Community Home Health at 317-621-4800 or toll-free at 800-404-4852, 24 hours a day, seven days a week.
