2010 annual report

2010 has been a challenging year in the healthcare industry and at Community Home Health (CHH). We integrated into one network, one employer, one team — Community Health Network, and we are very proud of our team. Through the exemplary direction of many individuals on the CHH board, we met the challenges of our industry by using innovative measures to improve efficiency and grow revenue. We recognized the need for integration and innovation with developing programming that crosses the continuum of care. Now, with one network and my role as the leader of network integration, we are better positioned to help improve the quality of care and quality of life for many patients and their families in our community.

From the newly diagnosed child with trisomy 18 to the elderly patient with multiple chronic diseases, we challenged ourselves every day to develop and provide better programs that meet and exceed our patients and their physicians’ needs. Programs like: telehealth, maternal-child, infant hospice, CPAP, online medical equipment and supplies, and senior care management services have been developed as part of our strategic plan while meeting our goals of providing the best care at the best level.

2009 annual report

The year 2009 was challenging for our nation and here in Indiana. As our nation grappled with two wars, worldwide economic failures and debate about health care reform, Community Home Health stayed focused on innovation, quality care, patient care transformations and increased efficiency throughout our organization.

Our patient outcomes have soared to be among the top in the nation. Many of our staff have received recognition for their outstanding care, and we have touched on some of those achievements in this annual report.

We were asked to develop a new service for some of our most fragile patients—apnea monitors allow mothers to bring their babies home from the hospital and be monitored in the comfort of their own home. Through the support of Community Health Network Foundation, we were able to purchase apnea monitors and additional telemonitors to enhance our home monitoring services. Our maternal-child program, launched in late 2008, enjoyed impressive growth in 2009, and with the availability of apnea monitors the growth became even more significant. In addition, we have developed hospice services for the very young and our hospice staff is busy planning more services to support our community.

2008 annual report

Every year, Community Home Health adopts a theme. Finding inspiration in Deborah Norville’s book, Thank You Power, we focused on thanks and giving in 2008—a positive foundation to support our commitment to delivering exceptional care. The giving spirit throughout our organization contributed to measurable successes in our outcomes, growth, awards and recognitions.

Our organization is full of people who truly care about our patients and the community around them, from a home health aide taking the time to facilitate communication with a patient using sign language so the home care nurse can deliver the best possible care to employees pulling together to collect supplies for mothers who would otherwise leave the hospital unequipped to care for their newborns. This giving spirit is the basis for the high-quality care we provide at Community Home Health.
