2011

Community Home Health outcome report

According to the quarterly Medicare Home Care Compare report, Community Home Health has been recognized for outstanding patient outcomes. Community Home Health exceeded the national average in 19 out of 21 published outcomes, while ranking in the top 20% in the nation in five of those outcomes. Medicare monitors home care patient outcomes to determine the percentage of patients showing improvement at the end of their episode of care. The outcomes are then published four times annually on the Home Care Compare website, comparing agencies to the national average and to other home care organizations throughout the nation. 2010

Community Home Health Services (CHHS) named a 2010 HomeCare Elite Top AgencyCommunity Home Health named a HomeCare Elite top agency. Now in its fifth year, HomeCare Elite identifies the top 25 percent of home health agencies in the United States and further highlights the top 100 and top 500 agencies overall. CHH is recognized for being among the nation’s top 25 percent of agencies. Winners are ranked by an analysis of performance measures in quality outcomes, quality improvement and financial performance.

Medicare published reports shows Community Home Health is in the top 20 percent of the nation in most patient outcomes, and that many outcomes are in the top 10 percent of the nation.

Compliance in the use of medical equipment is very important and especially with sleep apnea equipment. Our CPAP compliance in 2010 was documented as 86 percent, which is above the nationally published compliance rates of 50 to 60 percent.

Melanie Parks, R.N., hospice nurse, was among the 2010 Community Health Network Nursing Excellence Award recipients. Her award was for patient-focused care.

The third annual Celebration for Life campaign was just in time for Mother’s Day. Together with Community Health Network, essential items were collected for new moms who go home from one of the network hospitals without basic essentials for their babies. The employees of Community Home Health purchased a variety of baby items including bottles, blankets, clothing, etc., that were placed in 200 diaper bags and distributed to new moms by network hospital case management.

Angie’s List awarded CHH an “A” rating in health. Angie’s list is a consumer-based organization that collects ratings on health care providers in more than 140 categories.

In the fall, Shirley Brothers Crematory and Mortuary acknowledged CHH as a community partner by hosting a golf outing at Pleasant Run Golf Course. More than 45 vendors and golfers enjoyed a day of golf and lunch with all proceeds going to Community Home Health and our hospice services. The fundraiser outing was another way we integrated into the community we serve.

2009

Community Home Health (CHH), part of Community Health Network, received the “Innovative Program Strategy 2009” award from Philips TeleHealth Solutions this month due to the innovative use of telehealth for home care patients. The award focused on CHH work with chronic obstructive pulmonary disease patients. Learn more about telehealth services >>

In a year when many organizations were cutting back on continuing education, Community Home Health was able to provide health care professionals with specialized training for geriatric care, thanks to a $70,000 grant from Pfizer Pharmaceuticals. CHH, in partnership with the Indianapolis Discovery Network for Dementia (IDND), organized a multi-session summit on cognitive impairment, to improve delivery of dementia care. 2008

  • Jessie Westlund, CHH chief executive officer, was one of the top 5 finalists for the national 2008 Award for Excellence in Healthcare Leadership. The selection committee, composed of executives across The Advisory Board Company, stated that as a finalist she demonstrated superior organizational impact and leadership growth.
  • 2008 Consumer Choice Award for Home Nursing was received after several consumer surveys recognized CHH as their choice for home nursing.
  • David J. Schulte, CHH chief financial officer, received the Healthcare Financial Management Association Founders Medal of Honor Award in recognition of outstanding leadership and service to the state and national organization and to the development of the profession.
  • Received the Home Health Distinguished Award of Excellence from Health Care Excel, the Indiana Medicare Quality Improvement Organization.
  • 2008 Nursing Excellence Awards were received by two of our nurses: Katherine Quesenberry, Flu Clinic R.N. and Stephanie Wilson, Hospice R.N.
  • For the second year in a row, CHH was nominated for the Fishers Chamber of Commerce Pillar Award for Business of the Year. Fishers Chamber is the fastest growing chamber in the nation.

2007

  • Health Care Excel recognized Community Home Health as one of four home care organizations in Indiana who received both the Award of Merit for Quality Improvement and the Health Care Excel Quality Improvement Award.
  • Lori Beresford, MSW, won the Cheryl Atwood Cargiver Award, given by the Indiana Association for Home & Hospice Care (IAHHC). This award honors one caregiver each year that makes a significant positive impact on patients or staff or community and provides leadership within the organization.
  • CHH patient satisfaction scores have been great and the organization continues to outperform itself in the areas of overall experience, quality and safety.
  • Largest home health company in Indiana.
  • Provider of home care, home infusion, respiratory, home medical equipment and hospice services throughout central Indiana for more than 50 years.
  • Named a "HomeCare Elite" agency — ranked in the top 25 percent of all home care agencies in the nation based on quality, improvement and financial performance.

2006

  • Jessie Westlund, R.N., CEO of Community Home Health, received the Norah J. McFarland Award, the highest award from the Indiana Association for Home & Hospice Care (IAHHC).
  • Health Care Excel recognized Community Home Health in many areas: for success in decreasing acute care hospitalizations, its outstanding outcomes rate, for sustaining improvement, for improvement in outcome measure of improvement in dyspnea, and an award of merit for improving shortness of breath. 
  • Named a "HomeCare Elite" agency — ranked in the top 25 percent of all home care agencies in the nation based on quality, improvement and financial performance.
  • CHH recognized as Indiana's largest home health agency with 80,390 visits recorded in 2006. 
  • Four CHH nurses (two in home care and two in hospice care) were nominated for honors in the 2006 Salute to Nursing recognition sponsored by the Indianapolis Star.
  • Three CHH employees chosen as Community Health Network Hot Shots. Hot Shot awards recognize employees who provide exceptional customer service, either to outside customers such as patients or to internal customers such as coworkers.
