Touchpoint: Healthy Life Transitions
Community Health Network is taking an innovative approach to health care for patients age 55 and older. Touchpoint will bring together clinical and retail health care resources to offer healthy life transitions at all levels of care.
“We named the program Touchpoint because we are providing answers,” says Jessie Westlund, R.N., CEO of Community Home Health Services and director of Life’s Journey and Touchpoint. “We want to reach out to people age 55 and older and point them in the right direction when they have questions.”
Touchpoint brings many of the network’s existing services, along with some new ones, together under one umbrella. On the clinical side, the Seasons unit at Community Hospital North will serve as the behavioral care component of the program; Life’s Journey will work with patients managing a chronic illness; and Community Home Health Services will facilitate dementia care and offer Telehealth services. Finally, Community Physician Network physicians will be tied into the Touchpoint continuum of care.
“We will continually educate and communicate with physicians because they are the first point of contact for clinical care,” says Westlund.
Retail services will include an online membership tied into the myCommunity program, where Touchpoint members will have access to a variety of services as well as social opportunities, clubs and future travel opportunities. At Wellspring Pharmacy, Touchpoint members will have special discounts through the pharmacy’s Silver Club, and Pro Health will be offering or making available Medicare Advantage products. Touchpoint will also encourage members to engage in their health through special programming including classes and lunch and learns, and with regular newsletters and wellness topics.
“We want people to have support if they have health issues, or if they just want to stay healthy, and to evolve our relationship with them as their needs change” says Westlund.
