From I-69 and 82nd Street, exit and go south on Shadeland Ave. You'll pass Community Heart and Vascular Hospital on the left, and pass the Golden Corral Restaurant on the right. Take the second right and the entrance is in front of you, to your left. Our address is on the windows and our sign says: Community Health Network, Health Promotions, Employee Assistance.

Community Hospital East

Community Hospital East is located on the southwest corner of 16th Street and Ritter Avenue (Ritter is between Emerson Avenue and Arlington Avenue). From I-70, exit at Emerson Avenue and go south to 16th Street. Turn left on 16th Street and take the first right onto the hospital campus, just after the East Side Prescription Shop. Park in lot immediately to your left (before reaching the ramp down to the receiving dock). If lot is full, there is another parking lot to your immediate right (west). The first door on the building says Materials Management/Purchasing. Walk past that door to the right, and enter the door around the corner to the left, which says Occupational Health and Employee Assistance Program. After hours when the door is locked, push the button on the intercom in the vestibule, and the therapist will give you access into the building. The EAP waiting room will be on your right, just after you enter the door.

Community Hospital South
1402 E. County Line Rd. South

EAP clients should utilize the hospital’s main entrance at 1402 County Line Road. The hospital’s Emergency Room entrance is located to the left (west) of the hospital’s main entrance. Upon entering the building (main entrance) proceed past the information desk (located on the left), and follow the hallway as it bends to the left. Clients will see the hospital’s Bamboo Café on the right, but should proceed past this area to the metallic silver colored elevators on the right. Clients should use these elevators to reach the 2nd floor. Upon exiting the elevators, clients will view the area straight ahead where there is a smaller-sized desk with a hospital telephone located in the small waiting area. Clients will also see a sign providing instructions for calling the EAP therapist. Clients can use the waiting area located to the right of the elevators near the ICU (Intensive Care Unit) waiting room. The EAP therapist will escort clients to their office at their scheduled time.
