Request a consultation

When requesting a consultation, you may:

• Provide the patient with our phone number and he or she can call our office directly.
• Fax us an order with patient contact information and we will call the patient directly.
• Utilize secure clinical messaging and e-mail consult request and we will call the patient directly.

Please note:

• In all cases, patient medical records are required from referring office prior to appointment time and may be faxed to our office.
• Please send, or have the patient bring, actual MRI/X-ray images (films or CD) if completed outside Community Health Network.
