To schedule an appointment at Community Hospital South, Community Imaging Center South, or one of our other south imaging locations, please call 317-497-6320 or toll-free 877-298-8177.
Same-day appointments and walk-ins are welcome at Community Imaging Center South.
When you call, we will ask for the following items:
- Date of birth
- Social security number
- Insurance carrier and information from your insurance card
- Diagnosis
- Name of the test your doctor ordered
Patient forms and exam guides
You may download and fill out patient forms ahead of time from the downloadable forms page. You will also find helpful exam guides that provide an overview of the exam, what to expect and patient preps (if necessary).

If your doctor’s office has already scheduled an appointment, you may pre-register by calling 317-355-3920 or 877-217-7355 (toll-free) or pre-register online.
