How to apply for the pharmacy residency program
If you are interested in applying for the Community Health Network PGY1 residency program, please utilize PhORCAS to submit all application materials. These will include a letter of intent, curriculum vitae, three letters of recommendation, and official college transcript(s).

Application materials should be received no later than January 7, 2013. Upon evaluation of application materials, qualified applicants will be offered an on-site visit and interview with the residency team. Interviews take place in late January and early February. Successful candidates will be expected to graduate with a Doctor of Pharmacy degree, have an active BLS certification and obtain Indiana licensure by August 1. The application process will include a formal employment application to be completed upon successful matching with our program.
