Asthma management
The Bridges to Health asthma education and
monitoring program is designed to help you or
your loved ones better manage asthma.
Historically, it was thought that asthma was an
intermittent illness that was characterized by
episodes of wheezing, shortness of breath, chest
tightness or coughing. It is now known that asthma
is a chronic disease and that airway inflammation
is an early and persistent feature of the illness. If
left unchecked, this inflammation can lead to
permanent structural damage in the lungs,
resulting in irreversible breathing difficulties.

Cholesterol management
The Bridges to Health cholesterol education
and monitoring program is designed to help
you or your loved ones better manage
cholesterol. In the United States, approximately
one person every minute will die from a heart
attack. Studies have shown that elevated levels
of low-density lipoprotein (LDL) cholesterol is
a major risk factor for heart disease and it can
be lowered with diet, exercise and medications.
It is estimated that up to 90 percent of patients
are not being treated optimally, and therefore
are at risk for a heart attack.

The Bridges to Health diabetes education and
monitoring program is designed to help you or your
loved ones better manage diabetes. Studies
have shown that patients who tightly control their
diabetes early in its progression have much better
outcomes than patients who don’t achieve good
control of their disease.
